Pet Owners Receiving ODSP
You must show the veterinarian a recent ODSP Income Statement from the Province. The statement should be no more than two months old.

If you do not have a current ODSP Income Statement, contact the nearest regional ODSP Office and ask the office to send you a letter verifying that you receive the ODSP payment.
